RF Ablation

RF ablation is indicated for the treatment of symptoms due to urinary outflow obstruction secondary to benign prostatic hyperplasia (BPH). The treatment is most successful for men over the age of 50 with prostate sizes between 20 and 50cm3.

Overview

Examples of prostate shapes that can be treated with RF Ablation*:

20-50 grams Long Lobes Short Lobes Asymmetric Gland Median Lobe

*The PROSTIVA RF System is indicated for the treatment of symptoms due to urinary outflow obstruction secondary to benign prostatic hyperplasia (BPH) in men over the age of 50 with prostate sizes between 20 and 50 cm3. Prostate lobes of varying sizes and shapes can be treated, including asymmetrical prostates and median lobe (except the median lobe that grows into the bladder and collapses across the bladder).

Candidates for RF ablation meet the following criteria**:

  • Exhibit moderate BPH symptoms
  • Dissatisfied with medication results or side effects
  • Concerned with cost and inconvenience of lifelong medical therapy
  • Unwilling to undergo surgery due to concerns about anesthesia or recovery time
  • Concerned about impotence, incontinence, and other medication or surgical side effects
  • Not appropriate candidate for surgery

Patients with the following conditions should not be treated**:

  • Active urinary tract infection
  • Neurogenic, decompensated, or atonic bladder
  • Urethral strictures or muscle spasms that prevent insertion of the cartridge sheath
  • Bleeding disorders or patients taking anticoagulation medication unless antiplatelet medication has been discontinued for at least 10 days
  • ASA class group V patients
  • Clinical or histological evidence of prostatic cancer or bladder cancer
  • Prostate gland which is less than 34mm or greater than 80mm in transverse diameter
  • Presence of any prosthetic device in the region that may interfere with the procedure
  • Patients whose prostate has been previously treated with non-pharmacological therapies
  • Presence of cardiac pacemaker, implantable defibrillator, or malleable penile implants
  • Patients with any component(s) of an implantable neurostimulation system

Patient Selection Materials

The American Urological Association (AUA) BPH Symptom Score Index was developed to help patients assess the severity of their BPH symptoms. This short survey may be used to help your patients discuss their symptoms.

International Prostate Symptom Score (IPSS)

The International Prostate Symptom Score is the AUA Symptom Score Index, plus an additional question on quality of life as a function of urinary symptoms: “If you were to spend the rest of your life with your urinary condition just the way it is now, how would you feel about that?”

This presentation discusses BPH diagnosis and treatment algorithm:

  • Initial evaluation
  • Diagnostic tests
  • AUA Symptom Score
  • IPSS Score
  • Bother Score
